There will be (2) 45-minute breakfast roundtable discussions on a myriad of member-submitted topics ranging from open discussions, to specific departmental updates, or focused on networking.

Faculty have experienced significant differences in expectations from their own among contemporary learners regarding orientation to a career as a doctor, balance of the personal and professional, and preferences for learning methods. At a high level this is about the implicit social contract between faculty and learners. At an operational level it has to do with teaching methods suited to adults of generations different than faculty and finding new balances between things like accommodation and performance. Engaging New Learners is a work in progress that entails A) A department statement of philosophy about the social contract between faculty and learners; B) A toolkit of resources for effective pedagogy and faculty development; C) A spread or implementation model for local tailoring and use; D) Faculty development consultation modalities; and E) a tracking and evaluation function.

Specialty disrespect is “one element of the hidden curriculum, encompassing unwarranted, negative, and denigrating comments made by trainees and physicians about different specialties”(Allston 2019). Though negatively impacting our Departments, few know about it or have taken action to reduce it. We’ll define the issue and its impact, share experiences at our home institutions, and interventions that we tried. We’ll focus the discussion on opportunities for collaboration and next steps that Departments can take individually and together.

There will be (2) 45-minute breakfast roundtable discussions on a myriad of member-submitted topics ranging from open discussions, to specific departmental updates, or focused on networking.

This breakfast discussion will explore the valuable lessons that can be learned from failure. Through open dialogue and shared experiences, we can collectively identify strategies for overcoming challenges, building resilience, and driving innovation. This discussion will lean into defining failure, embracing vulnerability, identifying root causes, learning from mistakes, and building resilience. By understanding different perspectives on failure, creating a safe space for sharing experiences, analyzing contributing factors, extracting valuable insights, and developing emotional resilience, we can collectively contribute to the advancement of family medicine leaders. Sharing experiences will allow participants to contribute to a collective understanding of failure and its impact on professional growth. Those with less experience can gain valuable insights and strategies from the shared experiences of others. The discussion will reinforce community, support attendees, and nurture collaboration. This presentation aligns with the ADFM's values of excellence, integrity, inclusion, respect, and partnership. By creating a space for open dialogue and shared learning, we can empower each other to overcome challenges, build resilience, and drive positive change.

At this breakfast discussion, Chairs and Research Directors at two institutions will describe their programs that facilitate the integration of clinical and research missions. Participants will be invited to share their experiences with this challenge as part of the group’s learning. 15 - Topical Breakfast Tables: Making Morbidity and Mortality Meetings More Meaningful—a Focus on Implementing Continuous Quality Improvement
Wednesday February 19, 2025 7:00am - 8:30am CST
There will be (2) 45-minute breakfast roundtable discussions on a myriad of member-submitted topics ranging from open discussions, to specific departmental updates, or focused on networking.

Previously, Morbidity and Mortality conferences (M&M) at our institution were simply sessions where cases that had a bad outcome were presented and blame was often assigned, with little productive conversation about process improvement. Residents and faculty often left those conferences feeling guilty and more stressed. We have refocused our Morbidity and Mortality conference into one that centers on practice and quality improvement. In our new approach, each case is presented and all cognitive and system issues are addressed with a Fishbone Diagram, with a significant amount of time devoted to the discussion of identified problems and potential solutions. We believe this discussion is the most important part of the presentation. We continually stress our philosophy to our residents that these processes are not meant to be punitive, but are meant to promote self, departmental and systemic introspection to continually build a safe culture for learning and patient care improvement. Our department’s change has led to better teaching opportunities in patient safety and quality improvement to meet ACGME program objectives and to continuously improve the quality of our patient care. With our interventions, we have sought to foster this philosophy in not only our inpatient care but, equally importantly, our outpatient care too. We hope to inspire similar adoption of this process at other academic and non-academic practices.

Implementation of a Manager, Medical Education (MEM) leadership role for Family and Community Medicine to provide over-arching educational oversight for the department; focusing on clerkship (pre-doc), residency, and fellowship. The MEM leadership role assists in facilitation of key department and school of medicine initiatives, acts as a liaison for the department and GME office, resource development, and broad-spectrum management of all Family and Community Medicine's educational aspects. The Manager, Medical Education is integral in fostering an environment of cross-collaboration, effective communication, and the ability to breakdown silos that can prove to be detrimental in the progress needed to drive change and innovation. This role is the true pathway to ensure we are providing an exceptional experience to our learners all the entire way through their educational journey, with the goal to be able to retain them as faculty.

Innovation Showcase - A Faculty Onboarding Coaching Program
Wednesday February 19, 2025 1:00pm - 1:30pm CST
In 2021, the Department of Family and Community Medicine added a Coaching Program to our new faculty onboarding approach. Each new faculty is assigned a designated coach from among experienced faculty. Over the course of 6 months, 15 hours of dedicated coaching time is blocked set aside for 1:1 coaching sessions. There is a short list of topics that are expected to be covered during these sessions. Most of the time is unstructured so that the new faculty can set the agenda to cover topics they perceive they need help with. This program has proven very successful and popular with new faculty.

Innovation Showcase - Fueling Future Scholars: The UWFMR Research Track (ResT) Experience
Wednesday February 19, 2025 1:00pm - 1:30pm CST
The University of Washington Family Medicine Residency (UWFMR) Research Track (ResT) is a pioneering initiative designed to enhance residents' research knowledge and productivity, introduce residents to the academic medicine structure at UW, and provide tools for launching a career as physician-scientists post-residency without extending the length of residency training – a crucial part of the larger pathway to physician-scientists critical to our specialty. It also aims to enhance the quality of scholarship within the UWFMR program and encourage cross-departmental collaborations with an emphasis on resident scholarship. This innovation session will provide an in-depth overview of how ResT operates within the UWFMR program. This includes details about longitudinal structured research time, mentorship, and financial support for national conference attendance. Attendees will learn about the impressive achievements of ResT residents, such as their conference presentations and peer-reviewed publications, and gain insight into the career trajectories of program alumni. We will also share key factors contributing to the program's success, such as robust support from department leadership, inter-departmental collaborations, and dedicated resources. Additionally, opportunities for improvement, such as enhancing designated research staff support and increasing funding, will be discussed alongside future plans for program expansion through regional pilot projects. This session will highlight ResT as a replicable model for other Family Medicine departments aiming to bolster resident scholarly activities and promote a culture of continuous learning and academic excellence in family medicine residency programs.

Innovation Showcase - Creating a primary care-specialty care pathway from early detection to treatment for Alzheimer’s Disease
Wednesday February 19, 2025 1:30pm - 2:00pm CST
In 2022, through the Davos Alzheimer’s Collaborative we embarked on a combined quality improvement/research project using iterative quality improvement to implement early detection of Mild Cognitive Impairment in 7 primary care clinics within our academic health system. Over the next 12 months, we completed 1,808 screening cognitive assessments out of 17,894 encounters; completed an embedded research trial offering blood-based biomarkers; and developed and implemented a Brain Health Navigator role to assist with reversible cause evaluation and to facilitate transition to specialty care services, community resources, and clinical trials. Two patients identified in the primary care early detection program are currently on Disease Modifying Therapy, and 43 patients were referred to Alzheimer’s Disease and Related Dementia research studies. Further innovations are in progress with implementation of passive digital markers and automated subjective cognitive concern assessments.

Innovation Showcase - Innovating Healthcare Delivery: Innovative Residency Training in Correctional Healthcare
Wednesday February 19, 2025 1:30pm - 2:00pm CST
This presentation aims to focus on the family medicine physician's role while emphasizing innovation in healthcare delivery within correctional settings. Our Family Medicine department is structured uniquely to provide innovative learning opportunities. A key feature is the integration of daily clinical rotations at a county jail clinic, where both residents and faculty are actively involved to provide comprehensive acute and chronic care while also addressing the specific needs of obstetric patients. This setup allows our department to extend healthcare to marginalized populations while offering residents invaluable real-world experience. By engaging in these rotations, our department fosters a culture of hands-on learning, advocacy, and community service, preparing our residents to meet the diverse needs of patients in any clinical environment. Join us as we highlight the transformative impact of this program on residents, faculty and the community.

Harnessing AI in Clinical Practice: Opportunities to Enhance Operations and Patient Care
Friday February 21, 2025 8:30am - 9:45am CST
As Artificial Intelligence (AI) tools develop, leaders in academic family medicine need to be actively involved in shaping the way this technology is utilized in healthcare settings. While AI has great promise to contribute to more effective and efficient care, leaders must be well versed in opportunities as well as challenges associated with implementation, including potential bias and data privacy. This session will offer an overview of AI and machine learning, with a focus on applications in clinical and administrative operations. Attendees will hear from panelists who have started utilizing various AI tools within their departments to demonstrate lessons learned and future possibilities.

Innovation Showcase - Ambulatory Primary Care Staffing Renovation
Friday February 21, 2025 10:00am - 10:30am CST
This project began in planning phase in 2020. A team of clinicians, nurses, practice managers, directors, health IT experts, and data analysts aim to strategically increase staff support in clinics to achieve improved measures of practice performance. We calculated a positive return on investment and received approval from the health system for 3 pilot locations. The project “went live” in February 2023 in one family medicine clinic, one internal medicine clinic, and one pediatric clinic. We identified the following initiatives: increasing RN-led Medicare Wellness Visits, reducing appointment no show rates, increasing Transitional Care Management services, performing on quality measures. We also measure and monitor patient experience, care team function and satisfaction, access measures, productivity, patient portal utilization, and contribution margin via a dashboard created specifically for this project. Recognizing contribution margin as a key performance indicator, we partnered with a team member from business intelligence to build a new dashboard for volume and performance metrics. As of 10/30/24, we demonstrate pre-pilot net revenue per month of $499,073 and post-pilot net revenue per month of $548,733. Contribution margin per visit improved from negative $7 pre-pilot to positive $10 post-pilot. Our pilot was deemed successful and is now termed a project with spread to two additional family medicine clinic locations in July 2024.

Innovation Showcase - Implementing the Person-Centered Primary Care Measure Using an Existing Patient Experience Platform - Process, Findings, and Future Directions
Friday February 21, 2025 10:00am - 10:30am CST
The Person-Centered Primary Care Measure (PCPCM) is a patient assessment of their longitudinal experience of care with a clinician and care team, evaluating core functions of primary care in the health system. However, the optimal process of implementation across health systems, including how and when to administer the survey, reporting, and process improvement activities tied to survey data, has not been established. We distributed the 11-question PCPCM experience survey to 329,450 patients empaneled across 78 primary care practices between April 2023 and January 2024. We evaluated survey completion parameters, psychometric properties, and mean responses in relationship to patient-level demographic variables. In this large, heterogeneous system of primary care practices, the PCPCM survey was successfully distributed using the Press Ganey (South Bend, IN) platform. We found a low response rate (6.4%), but demonstrated good internal consistency, with a skew toward higher scores. PCPCM scores varied by age, sex, race, primary care provider type, and the number of years the patient had been at their current primary care practice. Responses varied significantly by patient race, but differences were small and not uniform in direction. Black or African American patients were significantly less likely to believe that the care provided by the practice was informed by knowledge of their community, compared with all other racial groups. The PCPCM was implemented successfully in a large network of primary care practices, but more work is needed to improve the response rate. Future work should focus on the use of the PCPCM for practice and provider feedback and validation of individual PCPCM items. Our plan for this session is to present our implementation process, briefly review our initial findings, and future directions. Our hope is that more health systems and primary care practices will implement this important measure of primary care quality.

Innovation Showcase - Inquiry: A Department-Wide Initiative to Support Staff and Faculty Curiosity, Wellbeing, and Scholarship
Friday February 21, 2025 10:00am - 10:30am CST
As our department rose out of the pandemic, leaders sought an initiative that would advance the development and wellbeing of all faculty whether they be clinicians, researchers, educators, or a mixture with an emphasis on non-research faculty. A two-day retreat revealed that the common wind in the sails of faculty members was curiosity. In response, our leadership team developed Inquiry over the past two years, a program that turns faculty and staff members' curiosity into deliberate and structured projects that can make a difference through sharing at the local, regional, national, and international levels. Our tag line: “Turning curiosity about what matters to us into inquiry that makes a difference to us.” Development of the Inquiry initiative has included creation of our Inquiry Portal, a centralized online location where requesters can share what they are curious about and ask for support in developing scholarship around their questions. To further this work, we have curated a team of part-time professionals including a research services specialist, a technical writer, a statistical data analyst, and two evaluation designers. Whereas the Inquiry Portal primarily accommodates requests narrow in scope (e.g., creation of a survey or manuscript assistance), the Pilot and Exploratory Program is staffed and funded to assist with multiple stages and needs of longer-term pilot or exploratory projects from start to finish which provides a small amount of funding and more research support for non-research faculty to advance their Inquiry projects.

Innovation Showcase - HOCUS POCUS - Developing Training & Credentialing for an FM Department in POCUS
Friday February 21, 2025 10:30am - 11:00am CST
The FM Department at the Zucker School of Medicine at Hofstra/Northwell created a GME POCUS program for the residents at the residency programs in the form of electives and a 12 week track (both ambulatory and inpatient). Simultaneously, the medical school had a UME curriculum for the students in POCUS starting from their first year. Upon the students entering the hospitals with their devices and entering residency programs, there was a need for credentialling of these individuals as well as the need for faculty to have the skills for oversight of new trainees. As such a standardized CME program was created along with delineation of privileges and credentialing guidelines.

Innovation Showcase - Leveraging Population Health and Value Based Care to Advance Each Mission of Your Department
Friday February 21, 2025 10:30am - 11:00am CST
Health systems’ business development processes have traditionally been designed around fee-for-service revenue opportunities. Health systems that are shifting towards value-based payment models must update how they assess business development opportunities including new clinical programs, growth, and optimization of resources. This presentation describes approaches that primary care leaders can leverage to advocate for business development processes that reflect the impact of value-based payment mechanisms on primary care services. The session sill include panel members discussing experiences leveraging value based care and population health to enhance clinical care, education, research, and health equity. The topic will be viewed from the lenses of academic departments, health systems, and payors.

Innovation Showcase - The Academic Leadership Academy: Preparing Leaders for a Bright Future in Academic Medicine
Friday February 21, 2025 10:30am - 11:00am CST
Physicians undergo rigorous didactic and clinical education over many years in order to practice their specialties at a high level. However, physicians typically do not receive training in the art and science of adult learning theory and teaching, including instruction in leadership, mindsets, innovation, and adaptive challenges. Educational methods to most effectively impart this knowledge to future learners requires discipline-specific training and experience to develop this skill set to the fullest potential. The Academic Leadership Academy at the University of Tennessee HSC College of Medicine Knoxville has a curriculum that is designed to provide skill acquisition in several domains, including teaching and precepting, research and publications, academic career advancement and leadership. A cohort of 8-10 participants is chosen yearly through an application process that is open to all faculty of the UT College of Medicine Knoxville. Learning sessions include monthly two-hour Zoom meetings in the Faculty Development Seminar Series, and four in-person workshops of 5 hours each focusing on a deeper dive into leadership in Academic Medicine. Participants also work on a research project with opportunities to report on their progress throughout the year. Collaboration between departments is highly encouraged. All activities earn continuing medical education credit for a total of 75 CME credit hours for each participant through the year. Since the start of this program, all participants have been retained and are active in leadership in their respective departments. Many have progressed in academic rank, which is seen as a measure of the commitment to continued success of applicants after completion of the certificate program.

Friday February 21, 2025 11:15am - 12:15pm CST
While it can often feel easier to identify our challenges and focus on areas for growth, we should also reflect and celebrate the exciting work happening in our departments. Medical students and residents continually bring new perspectives into the discipline that we want to foster and guide as we work together to build a strong future for family medicine. During this session, current and recently graduated learners will share projects and innovative initiatives they led in their departments, with a focus on what opportunities in their department nurtured their leadership throughout the process.
